Hey Joe,

I haven't used FP in ages so I'm not sure how to do what you want with that - but you can use a service like www.jotform.com or www.wufoo.com to create a form, have the data emailed to you, and then redirect to a thank you page where you could offer your book to those who filled out the form.

However, if you plan on sending these folks a regular email newsletter, I'd highly recommend a service like aweber to handle signups and to send out your newsletters.
