Default how do i take a page from .pdf file and transfer

it to my word processor and then have multiple copies.

here's what i'm trying to do: on the last page
of the art of leverage is a great worksheet for
new projects. what i want to do is put in a word
processor as a document and then have a seperate
copy (filled out), for each project on my plate.

any way to do this?
